Are you registered in Medicare's PECOS? Any provider/entity who has submitted information to their Medicare carrier's Provider Enrollment department since 2003, is in the online system. To find out if you are listed:
(1) Click here for CMS's Ordering / Referring Provider Report
(2) Scroll to the bottom of the page and click on the Medicare Ordering and Referring File download link (it will take a few minutes to load, there are many pages).
(3) Check to see if your name is on the list (alphabetical, by last name).
If your name appears on the Report:
(1) Go to the PECOS Log in page at CMS
(2) Follow the online instructions for access. You will need your NPI user name and password to access PECOS.
(3) Review the information to make certain it is accurate.
If your name DOES NOT appear on the Report:
(1) You are required to submit a completed Medicare application, by paper document or online at PECOS.

If you are in need of professional assistance to complete the online application, download the CLIENT AGREEMENT FOR PECOS CREDENTIALING (below). Upon receipt of your signed agreement, deposit, confirmation of email address, and required starting documents, the PECOS application can be underway.
